There was a serious incident in a marine terminal. After a CFS operation, a container was prepared to lift with Reach Stacker. Meanwhile, a checker and an intern wanted to control the seal on the container. But unfortunately they turn their back to Reach Stacker and they could not see it’s back maneuver. Reach Stacker operator did not see them during back maneuvering so RS hit to intern and her leg was injured under the wheel of Reach Stacker. She was taken to the hospital for surgery operation after the accident.

 Risk assessment should be revised and blind spots of heavy industrial equipment should be defined and marked clearly.
 New RFID solutions can be used to prevent accidents and incidents caused by the collision between pedestrians and equipment. RFID sensors can be fitted to equipment to make warnings and also it can be carried by a pedestrian on jacket or helmet to make warnings.
 All heavy industrial equipment should have reverse cameras and radar sensor to detect pedestrians or any obstacles for preventing collisions. All sensors and RFID equipment should be tested periodically.
